Effective March 1, 2024 Sunrise Pediatrics has retained Storr Records Management to be custodian of our medical records for the next 3 years. Storr Records Management does not handle billing, medication refills or referrals for the closed practice.

To obtain your medical records from March 29, 2024 onward:

1. Download the new authorization form (there are 3 pages) from our website. Click here to download.

2. Email form to requestfile@storrrecord.com. You will need to include a photo ID when submitting the records request.

Due to HIPAA guidelines this method is the only method to access your child's records.
